This prospective observational study evaluated acute maternal blood pressure response and short-term fetal Doppler findings after a single dose of sublingual nifedipine in third-trimester hypertensive pregnancies. The study included 30 singleton pregnancies in the third trimester with systolic blood pressure ≥150 mmHg and diastolic blood pressure ≥100 mmHg. Each participant received a single 10 mg sublingual dose of nifedipine. Maternal blood pressure, pulse rate, fetal heart rate, and umbilical artery and middle cerebral artery Doppler indices were measured at baseline, 30 minutes, and 60 minutes after administration. Data were analyzed using one-way repeated-measures analysis of variance. Maternal systolic and diastolic blood pressure decreased significantly after nifedipine administration (p<0.001), while maternal pulse rate increased significantly (p<0.001). Fetal heart rate showed no statistically significant change across the three time points. No statistically significant short-term changes were observed in umbilical artery or middle cerebral artery Doppler indices during the 60-minute observation period. During this period, no acute maternal adverse events requiring intervention and no signs of fetal distress were observed. In this cohort, a single 10 mg dose of sublingual nifedipine was associated with a significant acute reduction in maternal blood pressure. Fetal heart rate and the reported umbilical artery and middle cerebral artery Doppler indices showed no statistically significant short-term changes during the 60-minute observation period. These findings should be interpreted cautiously because of the small sample size, absence of a comparator group, lack of neonatal outcome assessment, short follow-up duration, and pierced-capsule sublingual route.
